Per the Illinois Uniform Peace Officers’ Disciplinary Act, 50 ILCS 725, a person filing a complaint that is made against a sworn officer must use this sworn affidavit form and have it notarized:

The Cook County Sheriff’s Community Inspector General Unit (CIG) serves as the Inspector General for a number of municipalities within the Cook County area, where it investigates complaints of theft, fraud, waste, and misconduct by elected officials and employees within suburban municipal governments. The CIG reports their findings to the elected officials of the governing agency, and when appropriate, makes referrals for criminal charges to prosecuting authorities such as the State's Attorney's Office or Attorney General’s office. In addition, the CIG conducts operational reviews and audits on policies, procedures, and protocol to assess efficiency and to determine compliance with policies, statutes, judicial orders and consent decrees. Requesting the Sheriff’s Office as a town’s Inspector General ensures that a fair, unbiased, and professional investigation will be conducted by an agency with no conflict of interest regarding the outcome of the complaint.

For non-IG related towns’ complaints, the CIG will review those complaints and will either investigate or forward them to the appropriate investigative unit for follow-up. The complaint form for all municipalities is available here:
